SLIPS (Southwark & Lambeth Integrated Care Pathway for Older People with Falls) is an integrated falls service across health, social, voluntary and leisure sectors in Southwark and Lambeth.
Under a standardised care pathway, SLIPS aims to deliver a consistent service across the two boroughs. All patients passing through the service receive the same access to falls assessment and treatment, and are provided with individually tailored falls management programmes to suit their specific needs.
SLIPS is community focused and patient centred. It strives to improve the patient journey by minimising waiting times and reducing the number of falls assessments that need to be carried out by different healthcare professionals.
The service endeavours to reduce the number of people having falls throughout Southwark and Lambeth by providing evidence based rehabilitation and preventative strategies for those who have fallen and/or are at risk of falling.
This website aims to provide information on falls and falls services across Southwark and Lambeth. It is designed for use by clients, carers and their families, and by health and social care professionals to facilitate appropriate, timely and evidence based management of fallers and those at risk of falling.
The website is divided into two simple to use sections; one for clients and non-healthcare professionals, and one for healthcare professionals to access. It includes hyperlinks to enable easy access to appropriate referral forms and links to other parts of the website.
